it's interesting that this seems to be happening to a few people. I wonder what makes the pulley drift forward when it comes loose? I would have thought that it would have drifted backwards until it hit something? Or is the shaft quite short so it can only drift forwards?

My theory is that, the pulley is spinning anti-clockwise, so could have something to do with the nut coming undone, maybe through vibration and frivtion in the bearing.
When the nut comes loose, the tensioner bolt can pivot backwards slightly, and the pulley eventually works its way off...

old crappy bearing did it to mine.. kept on grabbing and some douche-bag in japan sprayed some kind of WD40 into the bearing to quiten it rather than replace the bearing... quitened it for all of about 2 mins..

for a $3.60 bearing get the new pully, and hammer out the old bearing and replace.. you MIGHT be able to use a 3SGTE altenator belt tensioner?? i think the 3SFE/5SFE have the same thing..

    Thanks for the info,

I just ended up buying another 4AGZE tensioner pulley instead of trying to find something that could fit it,

I think it would of have to have been the bolt the held the pulley on that has come loose, because before it fell off the belt was slipping when i revved past 6K rpm, so maybe it just got looser and looser until it gave way?
